The Condition Management Company is focused on delivering effective treatment to meet the individual needs of each client. Through our team of highly qualified and experienced clinicians, we work with each individual to design and deliver treatment packages focused on your needs and aims, putting the patient at the heart of service design. The Condition Management Company is led by clinicians with over 60 years in combined clinical experience and service design, across Nottingham and further afield. Our team have developed, operated and evaluated large scale services for the NHS and Department of Work and Pensions. We have also been involved in specialist service development, produced training programmes and provided performance consultation to other clinical services. Now we have taken the opportunity to focus on developing a local, personalised service with clients at the heart of the service.
