Our Medicare-certified home health agency, HomeCentris Home Health, offers skilled nursing and therapy services that can start immediately after a hospital discharge. Our Personal Home Care subsidiary can help a client transition to a daily care plan of personal care services for the Activities of Daily Living (“ADL”). Finally, our HomeCentris Community Care subsidiary can bring primary care right to our patient’s residence.
