Detailed care plans are customized to meet each person’s needs and may include varied services such as medical and dental care, therapies, home and personal care, medications, medical supplies, and essential transportation. For isolated or lonely seniors, warm meals and social activities at Hope Care Centers can be a welcome change. By staying within Hope’s continuum of care, these individuals can receive uninterrupted services, while avoiding unnecessary hospitalizations or emergency room visits.
