About Kinship PACE
At Kinship PACE, we believe older adults deserve the opportunity to live independently, safely, and with dignity in the place they call home. As a Program of All-Inclusive Care for the Elderly (PACE) provider, we deliver comprehensive, coordinated healthcare and supportive services for eligible adults age 55 and older. Our interdisciplinary team works together to provide medical care, rehabilitation, prescription medications, transportation, home care, social engagement, and other essential services—all centered around each participant's unique needs. Our mission is simple: to improve quality of life while helping participants avoid unnecessary hospitalizations and nursing home placement whenever possible. At Kinship PACE, we build meaningful relationships with participants, caregivers, healthcare providers, and the communities we serve.
