About Lakeland Care
Lakeland Care, Inc. (LCI) is a Wisconsin-based managed care organization (MCO) providing long-term care services and supports to eligible frail elders and individuals with physical and intellectual/developmental disabilities through Wisconsin’s Family Care program. Lakeland Care’s goal is to support individuals to live as independently as possible in their homes and communities by providing high quality, cost effective services and supports using a member-centered, outcome-based approach. Lakeland Care offers the Family Care Program in the following counties: Adams, Brown, Calumet, Columbia, Dane, Dodge, Door, Florence, Fond du Lac, Forest, Green Lake, Jefferson, Kewaunee, Langlade, Lincoln, Manitowoc, Marathon, Marinette, Marquette, Menominee, Oconto, Oneida, Outagamie, Portage, Rock, Shawano, Vilas, Waupaca, Waushara, Winnebago, and Wood.
