Paradigm Health launched in 2013 with a commitment to providing Indiana with premier hospice and palliative care through compassionate care and a holistic team approach. As a locally owned provider, we bring a new “paradigm” to home-based health care offering these services in a coordinated effort to provide and maintain exemplary care in our patient’s home environment. We employ the best of the best; our team members are driven, compassionate and take pride in the quality of their care and services.
