About PACE Southeast Michigan
PACE stands for Program of All-Inclusive Care for the Elderly. The support and services we provide allow older adults to remain independent in their own homes for as long as possible. Our interdisciplinary team approach is a unique model of care that allows our staff to personally get to know participants and their caregivers. Our team meets on a daily basis to proactively communicate and coordinate holistic care to prevent avoidable emergency visits, hospitalizations and nursing home placements, thus providing higher quality care. PACE Southeast Michigan serves Wayne, Macomb and Oakland Counties.
