About Presidium Health
Presidium Health delivers whole-person, wraparound care for individuals with complex medical, behavioral, and social needs. We help patients stay connected to care beyond traditional healthcare settings through mobile, in-home, and community-based care models. Our teams support patients in private residences, assisted living communities, skilled nursing facilities, and other settings where they call home. Our longitudinal model integrates clinical, behavioral, social, and care navigation support to reduce barriers, strengthen engagement, and support long-term stability. This includes Operation Care, Presidium’s structured approach to finding, engaging, and supporting high-need patients over time. Presidium partners with health plans, providers, skilled nursing facilities, community organizations, and other healthcare partners to support high-need populations through coordinated, patient-centered care that is embedded across local communities and care settings.
