Founded in 2010, Covenant Case Management Services (CCMS) is a CARF-accredited case management organization that serves individuals with intellectual and developmental disabilities (IDD) and traumatic brain injuries (TBI) across 56 North Carolina counties. Case management for IDD and TBI connects individuals to the services, resources, and care coordination they need to live safely and independently. A dedicated case manager works alongside the individual and their support network to develop a personalized care plan, navigate Medicaid Managed Care, and ensure access to the right services at the right time.
