myPlace Health was founded in 2021 by mission-driven healthcare leaders and organizations that are committed to drastically improving health outcomes, quality and experience for vulnerable older adults and frail seniors. myPlace Health is now a business division of SCAN Group. myPlace Health is an integrated care delivery organization that specializes in providing personalized care and coverage to older adults who wish to remain safely living in their homes and communities for as long as possible, rather than living in a nursing facility. Our mission is simple: to enable older adults to live the independent lives they deserve. The primary ways in which we pursue our mission are through capitated arrangements with government payers, such as Medicare and Medicaid under the PACE (Program of All-Inclusive Care for the Elderly) model, and as a value-based provider working in partnership with local health plans.
