SeeInMe is a secure, HIPAA-compliant continuity infrastructure platform designed to improve personalized care coordination for individuals living with dementia and other high-needs conditions. The platform functions as a portable โhuman context layerโ that travels with the person across home care, hospitals, senior living, skilled nursing, behavioral health, and community-based settings. While traditional EHRs capture diagnoses and clinical history, SeeInMe preserves the individualized behavioral, psychosocial, communication, routine, and environmental information that frontline teams and caregivers need to deliver truly person-centered care. Designed to work alongside existing clinical systems with minimal implementation burden, SeeInMe supports interdisciplinary care coordination through role-based access, secure sharing, and longitudinal continuity across fragmented care environments. The company is currently focused on dementia and high-needs aging populations, where transitions of care frequently contribute to avoidable escalations, caregiver strain, workforce inefficiency, and higher-cost utilization. SeeInMe has received more than $2 million in non-dilutive funding, including a National Institute on Aging (NIA) SBIR award and a Department of Defense-funded dementia care initiative. The company is also participating in the MassChallenge Health & Life Sciences Traction Program. Over time, SeeInMe is building the longitudinal human-context infrastructure layer needed to support more personalized, AI-assisted operational care coordination and continuity intelligence across the care journey.
